Joining The Practice

The area served by the practice covers the G44.3 and parts of G46, G76 and G77 post code areas, roughly from Muirend and Netherlee in the North and South Moorhouse Farm and Threepland Farm in the South.

The practice will register new patients resident within the area defined above.

To join the practice come to reception anytime between 9am and 5pm. Please bring your NHS card if you have one, or other forms of ID, eg driving licence, passport etc. You will be asked to complete a GP Registration form and health check questionnaire that will give us essential information while your records are being transferred from your previous Health Service Provider.  You can use the above links to download these forms at home.

Please bring both completed forms to the surgery along with your ID detailed above.

You will then be written to for a "new patient" appointment with Treatment Room Nurse for a health screening examination before seeing one of the GPs.

Under the new GP Contract, you will be registering with the practice, not a named GP, but you have the freedom to choose which GP you would like to see. Simply make known your preference at the time of making an appointment, but please remember that different doctors work different numbers of sessions, so on some occasions you may have to wait longer to see the doctor of your choice.
